Sport Uniform Hire

I understand I will need to pay a refundable deposit of $50 to hire these uniforms. If the uniforms are not returned in useable condition, with correct amounts or reasonable effort is not made to return within a timely manner then the Student Guild Sport Department has the right to revoke the return of the $50 deposit. I understand that I take full responsibility for these loaned items and will returned washed. I understand this form does not mean my rental has been confirmed. GUGC Sport will be in touch to confirm the loan.
